In accordance with House Bill 4224 and Texas Health and Safety Code Section 181.105, Hall of Fame Counseling provides this information to assist clients with requesting access to health care records, contacting the state licensing council, and filing a consumer complaint.

As a client, you have a right to request access to you or your child’s health care records. To request your records:
1) Submit a written request by email or through the secure client portal.
2) If you are the client, include your full name, date of birth, and specific records you are requesting. If your child is the client, include your child’s information as well and documentation verifying legal authority is required. Examples include guardianship documentation or medical power of attorney.
3) Any information requested for lawyers or divorce situations must be handled through a subpoena.
4) Requests may be subject to identity verification and applicable fees as permitted by law.
5) Records will be provided within the time frame required by Texas law.
